Support to rebuild connection, confidence and participation
MSSN programs are designed for people whose social participation has been disrupted by ageing, illness, disability, injury or major life change. Our programs can be matched to individual needs, goals and circumstances, helping people strengthen social connection, rebuild confidence and increase meaningful participation in everyday life.
Find the right pathway for the person
Each MSSN program responds to a different level of need, from rebuilding social connection and confidence through to more structured psychosocial support or help navigating major life and work transitions.
People do not all need the same type or intensity of support. MSSN programs are designed to meet different starting points—from needing a safe way to reconnect, to rebuilding personal capacity, to a more structured psychosocial support pathway or help through a major work and life transition.
CONNECTION & PARTICIPATION
For people who feel isolated, disconnected or less involved in everyday life and want a supported pathway back to meaningful social connection and community participation.
PERSONAL CAPACITY
An 8-week practical program for people who feel overwhelmed, stuck or less confident following illness, injury or disruption and want tools to rebuild routine, self-management and participation.
Structured psychosocial support
A 12-week capacity-building pathway combining structured activities, facilitated connection and practical support to increase social participation and engagement in community and, where relevant, work.
MAJOR LIFE TRANSITION
Transition to Work or Retirement
For people navigating a change in work status and the associated changes to identity, routine, confidence and social connection. Support is focused on building a practical next-step plan and sustainable participation beyond the transition.
Not sure which program is the right fit?
You do not need to select a program before referring. Tell us about the person’s current circumstances, goals and barriers to participation, and MSSN can help identify the most appropriate pathway
