MSSN works with organisations supporting people whose health, circumstances or life stage have reduced their social connection, confidence or participation.

Who to refer

A referral may be appropriate when someone is:

  • experiencing persistent loneliness or social isolation
  • withdrawing from usual activities or relationships
  • struggling to rebuild routine after injury or illness
  • transitioning into retirement or changed capacity
  • lacking confidence to reconnect with community
  • unable to access ordinary social opportunities without support

What happens after a referral

You do not need to select a program before referring. Tell us about the person’s current circumstances, goals and barriers to participation, and MSSN can help identify the most appropriate pathway

Referral

We receive the referral and relevant information about the person’s circumstances and goals.

Initial Contact

We speak with the participant to understand their needs, preferences and readiness for participation.

Program Matching

We identify the MSSN pathway that best fits the participant’s needs.

Participation

The participant receives structured support and facilitated opportunities to reconnect and to get started with their goals.

Review

Progress is reviewed against agreed goals and relevant outcomes.

Transition

Where appropriate, MSSN supports connection to sustainable community activities and ongoing supports.
