Stepped Care Psychosocial Support Program

A structured 12-week pathway to build social capacity, confidence and participation in community and everyday life.

When isolation, reduced confidence and disrupted routines begin to reinforce each other, people may need more than occasional social contact. This program combines facilitated connection, practical capacity-building activities and individualised support to help participants gradually increase meaningful participation.

When this pathway may be appropriate

This program is intended for people who need a more structured pathway to rebuild participation after illness, injury, disability or prolonged isolation.

Participation has reduced

The person is doing less outside the home or has stopped activities that previously gave structure or connection.

Confidence has declined

They may avoid situations that feel unfamiliar, demanding or socially difficult.

A stepped pathway is useful

They would benefit from gradually increasing activity and connection rather than moving immediately into work or independent community participation.

How the program builds capacity

Establish routine

Use regular activities and manageable goals to rebuild structure into the week.

Strengthen social confidence

Provide facilitated opportunities to communicate, contribute and connect with others.

Explore meaningful activity

Identify interests, hobbies and activities that can support motivation, enjoyment and participation.

Connect beyond the program

Identify community resources and realistic next-step activities that can continue after MSSN involvement.

The program is designed to create movement towards sustainable social participation rather than simply increasing the number of sessions attended.

Develop a practical plan for maintaining connection and participation beyond the 12 weeks.

For Referrers and Funding Partners

The 12-week format is particularly useful where the referrer wants a defined intervention period, agreed goals and a clear transition plan. MSSN can work alongside existing treatment, rehabilitation or support services where roles are clearly defined.
